Dear Parent/Guardian,

This message is for all parents and guardians of Grade 7 students.

Public Health nurses will be visiting [Devon Middle on 10/20/2026 to provide routine Grade 7 immunizations.

To help ensure a positive experience, we encourage students to:

  • Eat a healthy breakfast before coming to school.
  • Bring a snack to enjoy after their immunizations.
  • Wear a short-sleeved shirt to allow easy access to the upper arm.

Vaccines Offered

Eligible Grade 7 students may receive the following publicly funded vaccines:

  • Tetanus, Diphtheria, and Pertussis (Whooping Cough) (Tdap) Vaccine
  • Human Papillomavirus (HPV) Vaccine

Vaccination Decision Form Required

If you have not yet completed your child’s online vaccination decision form, please do so no later than 10/18/2026

A vaccine decision form must be completed for every Grade 7 student, regardless of whether the student will receive the vaccines.

Please note that vaccine decision forms submitted after this date may not be processed in time for the clinic.
